Financial Stability Program Evaluation Tools

NOTE: For each of these evaluation topics, there are 2 versions of the second page – They are the same EXCEPT that one includes EEET data, and one doesn’t. Use either version, but they differ as to the process of where to send the forms.

EEET Group Form with Financial Stability components --reflects the same comprehensive survey components but is formatted to fit the folded EEET Group 1 form. Gathers only “plan to” data. No before/after or demographics included.

Post Program AND time-lapse follow-up evaluations planned At the program

Instruction Script – For IRB compliance, this script assures that the survey process is voluntary and confidential. Read the portion “For planned follow-ups.”
ID Form for follow-up mailing – hand out at the end of a presentation or series if you intend to do a time-lapse follow up. It provides a format for determining a confidential ID# to match responses of the after-program and the follow-up surveys.

About 6 months after the program

Follow up mailing cover letter. Mail this with the appropriate follow-up survey. Be sure to include a self-addressed stamped envelope.

Where to send the forms…

Where you send the forms depends on which Financial Stability page 2 version you used, or whether you used the Group EEET form with the “plan to” Financial Stability components added to it. Please send them on as you collect them – at least quarterly.

A. Forms WITH EEET format (all versions)

1. Follow standard EEET procedures and send the evaluations ALONG WITH the completed EEET cover sheet (Link to http://www.ag.ohio-state.edu/%7Epde/pdf/EEETCoverSheet.pdf and a BIG self-addressed envelope to: Program Development and Evaluation (PDE), Attention: EEETs, Ag Admin Bldg, Room 6, 2120 Fyffe Road, Columbus, Ohio 43210

2. After EEET analysis, PDE will send your EEET summary to you and forward the originals and BIG self-addressed envelope to: Sharon Seiling, Extension Specialist, Dept of Consumer Sciences, 265 Campbell Hall, 1787 Neil Avenue, Columbus, Oh 43210-1295 for statewide data analysis and reporting.

3. Sharon will return your originals along with a frequency summary of your data for you to enter into your URS report.

B. Forms WITHOUT EEET format (demographics only)

1. Send them along with a BIG self-addressed envelope to: Sharon Seiling, Extension Specialist, Dept of Consumer Sciences, 265 Campbell Hall, 1787 Neil Avenue, Columbus, Oh 43210-1295 for statewide data analysis and reporting.

2. Sharon will return your originals along with a frequency summary of your data to enter into your URS report.
