For IRB compliance, the instruction script assures that the survey process is voluntary and confidential.
Use the Follow-up Mailing Form (page 2 of "script" link) for planned 6-month follow-up surveys.

- General: for more comprehensive programs. Items "f" through "s" are selectively repeated in each of the focused versions below. This is the only section that differs in each of the topic versions. The second page of each version is the same and incorporates Evaluation of Extension Teaching (EEET) and demographics.
